There are a number of tools available that can assist practices to examine their patient population health profiles. Currently the CHGPN has the following tools available:
- The Canning tool: this tool produces for practices population health profiles of people with asthma, diabetes and cardiovascular disease attending the practice. The tool also has an "Income Estimator". This feature demonstrates practice income potential from the various government incentives available for diabetes and asthma patients, and Cervical Smears and ATSI Health Checks as well. This tool works with Medical Director (2 and 3), Best Practice, Medtech 32 (6 and 7.5) and Medical Spectrum Classic. Version 4.8 is the most recent release of the Canning Tool.
- Practice Health Atlas: this provides a very thorough epidemiological analysis of patients within a practice by demographics and disease. In addition the Atlas has a business modelling section which can help practices identify gaps in their income. This tool works with Medical Director, Best Practice, Genie, Practix and ZedMed.
- Clinical Audit Tool: this tool enables practices to search for various cohorts of patients at their practice by disease, demographics or medications and can produce graphs and pie charts of populations of patients. This tool works with Medical Director, Best Practice, Genie, Practix and ZedMed.
- Central Highlands Online for Improved Clinical Engagement (CHOICE): The Network will be trialing a program which involved de-identified patient data for a number of chronic diseases being transferred from practices to a server at the Network Office. The data will be analysed, fed back and benchmarked to individual practices.
