CENTRAL HIGHLANDS GENERAL PRACTICE NETWORK PROGRAMS

Central Highlands General Practice Network receives funding from The Australian Government Department of Health and Ageing to support general practice locally. We are a proud member of The Australian General Practice Network (AGPN).

Aged Care GP Panels Initiative

MedicarePlus Aged Care GP Panels Initiative aims to ensure better access to primary medical care for residents of aged care homes and to enable GPs to work with homes on quality improvement strategies for the care of all residents.

Chronic Disease Management (CDM)

CDM in General Practice involves appropriate prevention, early identification and best practice management strategies.

Information Management In General Practice information management focuses on the better management of information that can assist in promoting improved clinical, patient and practice outcomes.

Lifescripts

This initiative supports General Practice to build on prevention activities in the areas of smoking, nutrition, alcohol, physical activity and weight management.
Practice Management Support GP members and Practice staff have access to telephone, email or face-to-face visits for any Practice Management issues and management support

Quality Use of Medicines

The QUM Program was developed in the context of the National Medicines Policy to improve the quality use of medicines in the community. CHGPN programs covers the National Prescribing Service (NPS) and Medication Management Review (Home Medicines Review).

Medication Management Review (Home Medicines Review)
This Program is the key national infrastructure to support the implementation of the Home Medicines Review. HMR is a consumer-focused, structured and collaborative health care service provided in the community setting to optimise quality use of medicines and consumer understanding.

National Prescribing Service (NPS)

The National Prescribing Service (NPS) provides accurate, balanced, evidence-based information and services to health professionals and the community on Quality Use of Medicines (QUM).

Mental Health includes: Better Access and Better Outcomes

Better access

This initiative aims to increase community access to general practitioners, psychiatrists, clinical psychologists and other allied mental health professionals for mental health care.

Better outcomes

Through the Better Outcomes in Mental Health care initiative the barriers to the delivery of quality mental health care in general practice have begun to be addressed in a structured and systematic way.

National General Practice Immunisation Program

This program aims to improve the quality of vaccination services provided by general practice, with the overall goal of improving immunisation rates Australia-wide.

Nursing in General Practice

The Nursing in General Practice Program focuses on the following two outcomes. One, to help build the capacity of Divisions to deliver support services for practice nursing, in particular to recruit and retain nurses in general practice and two, to coordinate and fund education and professional development opportunities for practice nurses in general practice.

Population Health includes: NPCC and Practice Data Management

NPCC The Australian Primary Care Collaboratives Program is a 3-year, $14.6 million initiative funded from the Focus on Prevention - Primary Care Providers Working initiative announced in the 2003 - 2004 Australian Government Budget. The Program is of international significance.

Practice Data Management

There are a number of tools available that can assist practices to examine their patient population health profiles. Currently the CHGPN has the following tools available: Practice Health Atlas, CHOICE and Clinical Audit. For further information regarding these tools contact Catherine Ferreira.

QI & CPD

The continuing professional development program aims to assist GPs improve the quality of care they provide to patients by supporting, and delivering the highest of standards of care to the community. There is greater emphasis on learner needs by means of various adult learning principles; aimed to improve GP attendances; and plan quality educational activities. It endeavours to provide the best speakers, in addition to, the latest, and most up-to-date, medical program delivery. The CPD education program is well planned and is developed in collaboration with the CPD coordinator, GP reference group and CHGPN Staff. This group convenes once a year to determine future CPD program delivery.
