Aged Care Access Initiative

The vast majority of Australia's older generation live healthy and active lives. In the future, the increased longevity of the population is the result of improved living conditions and healthier lifestyles, as well as significant medical advances. Even so, at some stage many will need assistance from an Aged Care service.

As the number of elderly people increases, so does the need to support them.
Australia's population is ageing. The number of residents of Residential Aged Care Facilities is increasing. There is also evidence that the proportion of residents of aged care facilities with high care needs and complex conditions is growing.

Both Residential and Community Aged Care services have been implemented by the Government.

The aim of the Aged Care Access Initiative is to improve access to primary care (GP and allied health services) for residents of aged care facilities.

Allied health services purchased under the Aged Care Access Initiative should not incur an out of pocket cost to the patient.

The CHGPN ACAI Model: 6 sessions per client to enable more sessions to go around.

  • RACFS and GPs have the opportunity to fax back a referral form to book a Mediation , transition into the RACF, grief and loss client, family and staff individual and group counseling session with a choice of 3 Allied Health Professionals- Psychologist, Geropsychologist and Socialworker.
  • Fax Back form is de identified- Initials of the client is only required.
  • Initial appointments are booked with CHGPN- subsequent appointments are organised with the Allied Health Professional.

The CHGPN support and recommend the "continuity of care" motto- feed back to the RACF staff and the clients GP is mandatory.

The RACF staff are required to complete an evaluation of the Allied Health Professional sessions once the sessions are completed.

By providing these funded sessions enables the resident to utilise their 5 Enhanced Primary Care (EPC) services to their chronic condition.

The uptake of the ACAI program has been positive, a significant amount of sessions delivered indicates the necessity and overall value of this program.
