The Department of Veterans' Affairs (DVA) new Coordinated Veterans' Care Program (CVC Program) commenced on 1st May 2011. Information regarding the CVC DVA Gold Card Holder Program is available at www.dva.gov.au/cvc Total 1st year incentive payment to General Practice to participate in the program is up to $2070 per patient.
The program is aimed at veterans, war widows, war widowers and dependants who are Gold Card holders and are at risk of being admitted to hospital. GPs who decide to be involved in the program are required to prepare for the program, enrol participants in the program and provide ongoing care. Gold Card holders DVA deemed eligible for the program will be notified by mail.
Australian General Practice Network (AGPN) in conjunction with the Australian Practice Nurses Association, (APNA) Flinders University and Baker IDI Heart and Diabetes Institute, have been engaged by the Department of Veterans' Affairs (DVA) to provide training and resources in chronic disease management for General Practitioners (GPs), Practice Nurses (PNs) and Community Nurses who will be implementing DVA's new Coordinated Veterans' Care Program.

In addition, the Network will deliver a local CPD/CNE 6 hour ALM workshop to be held on February 25th 2012 at the Bentinck of Woodend for GPs and PNs participating in the CVC multidisciplinary team managing patients with mental and physical health co-morbidities.
The Education and training packages have been accredited with RACGP, ACRRM and GPMHSC Mental Health Skills Training. RCNA and APNA endorsement is currently being finalised.
